Quest CHC is a not-for-profit, interdisciplinary, health care organization. They are one of 21 new government funded health centres. Their holistic model focuses on primary care, illness prevention, health promotion, community capacity building, and services integration.
With your skills, positive outlook and clinical expertise, you are a catalyst who will help support a new environment for residents who do not have a primary care provider and who commonly experience barriers to accessing healthcare.
Responsibilities:
- Work in collaboration with physicians, nurse practitioners and other allied healthcare professions
Qualifications:
- 3-5 years experience preferably in a community health setting working with inter-disciplinary teams, or combined with hospital and or public health environment
- Experience in program development, implementation, monitoring and evaluation is an asset
- Currently licensed and in good standing with the College of Nurses of Ontario
- A valid Ontario driver’s license, access to reliable transportation
- Police Reference check is required
